Consultations & Referrals
There are several ways to obtain home health or hospice services from Mount Evans. We can either answer your questions over the telephone, or have one of our registered nurses visit with you in your home to discuss our services. With you and your family, we will determine your specific needs, discuss medical supplies and equipment, and explain various methods of payment.
- Call 303-674-6400 to talk to a nurse or set up a consultation and we’ll notify your primary care provider; or
- Talk with your hospital, physician, or discharge planner and let them know of your interest in using Mount Evans Home Health Care & Hospice as an option for your home health care needs.
