Position Open – Nursing Manager for Home Health, Hospice and Palliative Care Programs
(Full Time)
We’re looking for a full-time Nursing Manager to support our mission by ensuring patient care is coordinated and managed appropriately for our Home Health, Hospice and Palliative Care Programs.
About the position
This position:
- Manages all direct and indirect patient services provided by clinical organization registered nurses, nurse practitioners, certified nursing assistants and personal care workers.
- Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing the plan of care.
- Maintains current knowledge of regulations and legal issues concerning home health care, hospice and palliative care.
- Leads interdisciplinary and discipline team meetings for home health, hospice and palliative care.
- Provides on-site supervision as needed to determine staff competency and responds to educational and developmental needs.
- Conducts performance evaluations annually, or more frequently when indicated.
- Develops and conducts annual clinical staff competency program.
- Effectively manages budget and costs associated with nurse, certified nursing assistants and personal care workers labor, supplies and activities.
- Participates in Agency quality assurance performance improvement activities.
- Coordinates employee health program, employee infection control and incident-reporting programs.
- Ensures Nurse On-Call coverage 24/7.
- Reviews chart documentation and counsels staff to ensure quality case management, records in compliance with local, state, and federal laws.
- Provides interviewing, hiring, and training of employees, planning, assigning, and directing work including goal and direction setting, appraising performance, rewarding, and disciplining employees, addressing complaints, and resolving problems.
- Is responsible for staff retention and staff development.
- Performs other duties as required and necessary to ensure the success of the Agency.
- Is based out of our office in Evergreen.
- Travels to homes throughout the foothills of Clear Creek, Gilpin, Jefferson, and Park counties, to provide patient care when needed.
- Has a salary range of $86,000 – $107,000 with access to a full benefits package.

About you
You must (required):
- Have an Associate’s or Bachelor’s Degree in Nursing.
- Have a minimum of two (2) years’ nursing experience in home health and/or hospice care.
- Have a minimum of one (1) years’ nursing management experience in a health care setting.
- Maintain an active Nursing license in the State of Colorado.
- Be able to demonstrate your ability to provide effective and compassionate nursing care.
- Be a self-managed leader with strong communication, organization, and time management skills.
- Have a thorough understanding of nursing management strategies and knowledge of financial management and the foundations of health laws.
- Provide documentation of full COVID-19 vaccination or documentation requesting a reasonable exemption for medical or religious reasons.
- Have a valid driver license, good driving record, proof of insurance, and reliable vehicle.
- Have proof of legal right to work in the United States.
Ideally, you will (preferred):
- Have one (1) year nursing management experience in home health care, palliative care or hospice.
- Have one (1) year home health care OASIS experience.
